What a call actually sounds like
A typical after-hours call to a clinic agent goes something like this. The agent answers within one ring: "Thanks for calling the clinic. I'm the clinic's AI assistant. How can I help?" The caller says they need to move Thursday's appointment. The agent finds the booking, offers Friday 10am or Saturday 11:30, and the caller picks Saturday. The agent confirms the change, mentions that a WhatsApp confirmation is on its way, and asks if there is anything else. Forty seconds, done, at 9:40 on a Tuesday night.
Notice what did not happen. No hold music, no "please call back during office hours", and no pretending to be human. The agent introduced itself honestly and then earned the caller's patience by being useful, which in our experience is the only thing callers actually grade you on.
How it actually works
Three parts run in a loop. Speech recognition turns the caller's words into text. An AI model decides the reply, usually grounded in your documents with RAG so it quotes your real prices and policies. A synthetic voice then speaks the answer. Platforms like Vapi complete that round trip in under half a second, fast enough that the conversation feels normal [2]. We pair it with ElevenLabs voices, which stopped sounding robotic a couple of years ago.
What we need from you to set one up
Less than most owners expect. The agent's knowledge comes from documents you already have: the price list, the FAQ, the opening hours, the rules your receptionist keeps in their head about what can be booked when. Getting those rules written down is usually the most useful meeting of the project, because half of them have never been written anywhere.
Then we connect the calendar so the agent can see real availability, agree on what it may and may not do (book and reschedule yes, discuss a patient's medical details no), and set up the phone side, which is usually just forwarding your existing number. Before any real customer hears it, your own team calls the agent for a week and tries to break it. Their trick questions become its training material.
